  • Gazette Notification Finally, you can complete a name change by using a Gazette Notifications through several documents like the Driver’s license, PAN Card, Voter’s ID, etc.

an affidavit will do the magic, but you have to publish change of name in 3 newspapers and the the changed name should be published in govt. gazette. any lawyer /typing services/notary should be able to do it. as far as time in concerned 5 days may be difficult, but you can re approach after the name change. by the way, what is the name appearing in your aadhar card (if you have one) and the name on your birth certificate issued by local authorities (if you are born after november 1989). these two documents are enough for getting a passport
